About the RoleAveanna Healthcare is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Care provider. In this role, you will have the opportunity to work with patients in their homes, providing one-on-one care and making a positive impact on their lives.Responsibilities

• Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
• Implement physicians' prescribed plans of care, including administering medications, treatments, and therapies.
• Conduct patient assessments and coordinate care with the healthcare team.
• Document all activities, assessments, and nursing actions in a timely and accurate manner.
• Participate in the development and implementation of the nursing care plan.Requirements
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.What We Offer
