About the RoleAveanna Healthcare is seeking a compassionate and skilled Private Duty Nurse - LVN to join our team and provide high-quality care to our patients. As a Private Duty Nurse, you will be responsible for delivering skilled nursing care to patients in their homes, working collaboratively with our multidisciplinary team to ensure the best possible outcomes.Key Responsibilities:

• Implementing and managing patient care plans, including administering medications and treatments as prescribed by physicians.
• Conducting thorough assessments and evaluations to identify patient needs and develop effective care plans.
• Providing education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
• Maintaining accurate and timely documentation of patient care, including progress notes, treatment plans, and medication lists.
• Collaborating with the healthcare team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.Requirements:
  • Active, unencumbered license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
