We match our nurses to their patients based on skill, age, health condition, and acuity level to ensure a seamless transition from hospital to home.

• Work with patients in San Antonio and surrounding areas
• Flexible scheduling options availableResponsibilities:
  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their familiesRequirements:
    • Active, unencumbered LPN/LVN license in the state of practice
    • Current CPR certification
    • TB skin test (current within last 12 months)
