We are seeking a skilled Physical Therapist to join our team at CHRISTUS Home Health, a part of LHC Group.As a Physical Therapist, you will have the opportunity to build meaningful relationships with patients and their families, while providing high-quality care and rehabilitation services.Key responsibilities include:

Evaluating patient functional status and physical therapy needs
Developing and implementing personalized therapy plans
Collaborating with healthcare teams to ensure comprehensive care
Providing education and support to patients and familiesRequirements include:
- Current Physical Therapy licensure in the state of Texas
- Current CPR certification
