• Certification Details
• Documentation of vaccination series completion OR documentation of declination to vaccine
• Headshot with a white or blue background.
• Vaccines or Positive IGG Titer. If Negative titer, must have booster vaccine. Exemptions are NOT accepted for persons working in the following areas: Potential contact with pediatrics/infants, non-immune pregnant women (L&D areas), non-immune immunosuppressed patients (oncology, transplant, etc.).
• Flu season starts Sept 4th, 2023 for all new travelers. Needs to be updated every year. Declinations and exemptions not accepted.

  • 18 month minimum Pediatric Cardiovascular ICU experience required.
  • Nurse : Patient Ratio by shift: 1:1, 1:2 for ICU status, 1:3 for stepdown/non ICU in overflow unit.

    • Certifications Preferred: ACLS
      • Additional Details
      • Must be AHA.
      • Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs but you will still upload here.
      • Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals. Nursys quick confirm verifications should be completed for all APRN, RNs and LPN/LVNs. Nursys should be used to review ALL license activity in all states, not just the state of placement. Copy of the physical license is not required. Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs.
      • Completed prior to start and updated every 3 years.
      • HIPAA, Joint Commission -NPSG, OSHA Topics
      • Exam and answer key found in Document Library. Agency is required to give and grade the test. Score needs to be 90% or higher with only 2 attempts.
      • Optional for Onboarding, but needs to be uploaded by our Ascension unit leader within a week.
