• Certification in Case Management is preferred but not mandatory.

  • Job Details
  • The Care Manager (CM) II collaborates with patient/family, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care.
  • Development of a patient care plan for each assigned patient from admission through discharge.
  • Responsibilities include comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
  • Care Coordination and Discharge Planning are roles of the CM.
  • The CM adheres to departmental and organizational goals, objectives, standards of performance, policies and procedures, and regulatory compliance.
    • Job Requirements
    • Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
    • Two or more years clinical experience with one year in the acute care setting preferred.
    • RN or LMSW in the state of employment is required for new hires.
    • LBSW accepted for associates with 5+ years in CHRISTUS Care Manager I role.
    • Certification in Case Management preferred.
    • BLS preferred.
      • Schedule Information
      • 8 hour days 8-5.
      • Works weekends with a weekday off in lieu.
        • Additional Details
        • The CM must have excellent verbal and written communication and interact with diverse populations.
        • Critical and analytical thinking skills are required.
        • Must have demonstrated clinical competency.
        • The ability to multitask and function in a stressful and fast-paced environment is necessary.
        • Working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
        • Understanding of pre-acute and post-acute levels of care and community resources.
        • Ability to work independently and exercise sound judgment.
